Credit Balance Check Form

RN to BSN Completion program is a 100% online program; therefore, students do not come to campus. If an RN to BSN student will receive a credit balance check, BTC will process through direct deposit. 

Students, please upload the completed form below and a copy of a voided check into your Populi account under Financial Aid, or mail to the address on the form. Parents, please email financialaid@bolivarcollege.edu for a link to access Populi and next steps.
